One form must be filled for each entry. 

For example, from one school, if there are three group entries for short film contest, three separate forms must be filled, and submitted.  In Speech Contest, a separate form must be filled and submitted for each individual contestant.

Maximum No. of entries allowed from each school are as follows:

1. Short Film Contest  - 3 group entries. 4 individuals in each group

2. Speech Contest  - 2 individual entries

3. Quiz Contest - 2 Group entries. 3 individuals in each group

4. Debate   - 1 Group entry. 3 individuals in the group

5. Med-Zap - 1 group entry, 3-5 individuals in the group
